This application must be completed in full before a permit can be issued. All information must remain current, and it will be the responsibility of the permit holder to ensure that this information remains current at all times.

**Attention residents 65 or older** are exempt from paying the alarm fee, however are still required to complete and mail an application. The application can be downloaded here: Alarm Permit Application.
