Please Note: By completing the form below, you are requesting a Duplicate Copy of a registration as a replacement for one that has been misplaced, lost, or destroyed. A Duplicate Copy is a sufficient replacement for normal daily operation of your vehicle.

A Certified Copy of your registration may be required for certain legal matters. Certified copies may only be completed at the Concord DMV Office. ?
